Your referral may be for ongoing care, a specific health concern, or a closer look at your current care plan. Contact our concierge team to arrange the next step with Alexander Acevedo, FNP-C.
- Contact the team. Let us know who referred you and which service you would like to discuss. You can also ask the team to help identify the appropriate starting point.
- Complete secure intake. The team will provide instructions for sharing your referral and relevant records.
- Confirm your visit. Initial consultations are $149. We’ll discuss availability and next steps before booking.

- Coordinate with Longevity Concierge Med. Send your practice name and a professional callback number when requesting referral instructions.
- Transfer records securely. Ask the team for the approved transfer method before sending clinical information.

Visit options & costs
We’ll confirm the appropriate visit format based on the service and your location. In-person care is available in Hawaiʻi, with telehealth options in Hawaiʻi and Illinois when appropriate. Initial consultations are $149. Any additional service costs are discussed before booking.
