PAYMENT & INSURANCE

HOW PAYMENT WORKS

Pay the practice directly.

Cash pay, also called self-pay or private pay, means you pay Longevity Concierge Med directly for an agreed service or membership. It describes the payment arrangement; it does not mean you must be uninsured or pay with physical cash.

Longevity Concierge Med’s membership dues and private-pay service fees are paid directly to the practice rather than billed by Longevity Concierge Med to an insurance plan. We confirm the fee, included services, and separate charges before you proceed.

DIRECT PRIMARY CARE, EXPLAINED

A membership for
your everyday care.

Direct Primary Care (DPC) is a recurring membership for a defined scope of primary care—everyday concerns, preventive visits, ongoing conditions, and follow-up with a provider who gets to know you.

It gives you an ongoing relationship with Alex under agreed access and service terms. Your membership agreement defines what is included, the fee, and the commitment.

DPC is not health insurance. Hospital care, emergency care, specialists, medications, and outside testing may fall outside your membership and carry separate costs.

PLAN FOR THE FULL PICTURE

Your membership.
Your other coverage.

Private-pay care can sit alongside other health coverage, subject to your plan’s rules and individual eligibility. Paying Longevity Concierge Med directly does not guarantee insurer reimbursement.

Outside labs, imaging centers, pharmacies, hospitals, and specialists have their own billing policies. Ask them and your insurer about coverage, network status, prior authorization, and your share of the cost before receiving a service.

The team will help clarify what belongs in your Longevity Concierge Med plan and what is arranged or paid for separately.

BEFORE YOU COMMIT

Know the expected cost.

Initial consultations are $149. Ask us to confirm membership dues, enrollment fees, the membership term, cancellation terms, and any additional testing or treatment charges.

Your right to a Good Faith Estimate

If you are uninsured or are not using certain types of insurance, you generally have the right to a written estimate of expected charges when you request one or schedule eligible care in advance. A qualifying bill that is $400 or more above a provider’s estimate may be eligible for dispute.

Timing, eligibility, and exceptions depend on the circumstances.

A CONVERSATION BEFORE ENROLLMENT

Questions are welcome.
Fit is reviewed together.

You can ask about Longevity Concierge Med regardless of your insurance or coverage status. An inquiry starts a conversation; it does not enroll you or confirm eligibility for clinical care.

When you request DPC enrollment review, we ask a brief coverage question so the team can consider membership suitability. “Yes,” “not sure,” and “prefer to discuss” all allow you to submit your request.

Medicare, Medicaid & enrollment qualification

If you have Medicare, Medicare Advantage, or Medicaid/Med-QUEST, the team needs to review applicable requirements before any clinical appointment or enrollment is confirmed. Cash payment alone does not resolve program requirements or establish eligibility.

Each prospective member is qualified individually. The website does not make an automatic approval or rejection based on your coverage response.
