YOUR INFORMATION
Website privacy
Last updated September 26, 2026
This policy describes information handled through this Longevity Concierge Med website. It is separate from the practice’s clinical Notice of Privacy Practices, treatment consents, and membership agreements.
Information you choose to provide
If you complete the starting-point questionnaire and submit its inquiry form, your four selected answers are included with your contact details for follow-up. How you heard about us and referral details are optional. If you submit a contact or enrollment inquiry, the website records your name, email, optional phone number, optional referral details, selected care interest, and request type. It also records a reference number and submission time so Longevity Concierge Med can review and respond to your request. Care interests may reveal health-related information.
Employer inquiries also record your organization’s name, number of employees, industry, and inquiry type, together with your contact information. These details help us respond to your organization’s request.
These forms are for administrative requests. Please do not send medical records, diagnoses, lab results, payment information, or urgent medical concerns through website forms or ordinary email. Use the clinical intake process provided by Longevity Concierge Med for medical information.
Waitlist emails
The optional waitlist collects your email address and, if provided, the state where you’d like to receive care, and records your consent wording, consent time, request reference, and signup source. We use this information to email you about waitlist availability and enrollment updates. It does not collect medical details or enroll you in care. You can withdraw consent by emailing info@conciergemed.com. A preference stored in your browser keeps the pop-up from repeatedly appearing; it does not store your email or replace your saved signup.
Your roadmap selections
Making selections in the interactive roadmap tools does not itself submit them to Longevity Concierge Med. If you separately send an inquiry, information associated with that inquiry is handled as described here. Opening a linked intake form or email application takes you to a separate service.
How information is used and handled
Longevity Concierge Med uses submitted details to respond to questions, discuss services, review member referrals, and coordinate next administrative steps. Submitting an inquiry does not automatically enroll you, approve a referral, or subscribe you to marketing.
Information is stored for Longevity Concierge Med’s review and processed by service providers supporting website hosting, storage, access, and security. Information may also be disclosed where required by law or necessary to address misuse or protect legal rights. Submitting a form is not consent to unrelated uses of medical information.
Testing and service preferences
Website inquiry forms do not collect optional test selections or free-text health preferences. Discuss these details through the secure clinical intake process provided by Longevity Concierge Med. Previously submitted information remains subject to the handling and request policies described on this page.
DPC enrollment qualification
General inquiries do not require a coverage response. In the DPC enrollment-review form, you may answer a brief Medicare/Medicaid question or choose to discuss it with the team. Any of these responses can be submitted.
When you submit that form, the response is used to mark whether the request needs further coverage discussion. The website stores that review status with your inquiry, not the specific answer as a separate coverage field. The response is not automatically saved in browser storage. Eligibility is confirmed through the practice’s individual review process. Do not provide insurance numbers or upload insurance cards here.
Hosting, fonts, and external services
Website hosting and access providers may process technical information such as IP addresses, browser information, authentication details, and security logs. Some pages load fonts from Google Fonts and interface resources from external content-delivery providers, including unpkg. Those requests can disclose technical information such as your IP address and browser details to the provider. This website does not currently include advertising pixels or a third-party marketing analytics collector.
External scheduling or intake services, email applications, and linked resources have their own privacy practices. Review those practices before sharing information there. Access or authentication cookies may be managed by the hosting provider.
Your questions and choices
For questions about information submitted here, or to request access, correction, deletion, or a stop to nonessential follow-up, email concierge@conciergemed.com. Longevity Concierge Med may need to verify your identity. Retention and deletion depend on the purpose of the record and applicable legal obligations; some information may need to be retained.
No internet service can guarantee complete security. Do not include sensitive health information in a privacy inquiry.
Clinical privacy and changes
This website policy does not replace a clinical Notice of Privacy Practices or describe every aspect of Longevity Concierge Med’s clinical record handling. Contact Longevity Concierge Med to request the practice’s clinical privacy notice and information about your medical-record rights. Website policies may be updated as services or data practices change; the date above identifies the current revision.
