CLEAR EXPECTATIONS
Website terms & medical disclosures
Last updated September 26, 2026
This website introduces Longevity Concierge Med’s care approach and possible service pathways. Individual clinical decisions, treatment consent, pricing, and membership obligations are established separately.
Information and emergency care
Website content and interactive gameplans are educational. They do not diagnose a condition, prescribe treatment, or replace an individual evaluation by a qualified clinician. Using a tool, sending an inquiry, or requesting a consultation does not by itself establish a clinician–patient relationship.
For a medical emergency, call 911 or seek emergency care. This website and its forms are not monitored for urgent medical needs. Do not delay appropriate care based on website information.
Testing and interpretation
Biomarkers, laboratory panels, preventive screenings, DEXA, VO₂ testing, imaging, and other diagnostics are examples of information that may inform care. They are selected according to clinical need and are not a universal testing package or a promise to detect every disease.
Testing can produce false-positive, false-negative, or incidental findings. Some imaging uses radiation. Exercise testing requires a review of suitability. Alex and the testing provider discuss the purpose, relevant limitations, and next steps for the tests recommended to you.
Prescription and treatment disclosures
Medical treatment and prescription decisions require an individual clinical assessment. Eligibility depends on the specific condition, medical history, contraindications, medication interactions, applicable law, and ability to provide appropriate follow-up. Benefits, risks, and alternatives are reviewed before treatment. A consultation does not guarantee a prescription or procedure.
An FDA-approved medication may sometimes be prescribed for an off-label use when lawful and clinically appropriate. Off-label use is different from FDA approval of that use and should be discussed in the individual treatment-consent process.
Compounded drugs are not FDA-approved. FDA does not review them for safety, effectiveness, or quality before marketing. Compounding is subject to legal limits and patient-specific considerations; it is not automatically an interchangeable substitute for an approved product. This website does not offer a specific investigational or research-use-only product for purchase.
Longevity and concierge care
“Longevity” and “Grow Young” describe a focus on long-term health, function, and quality of life. They are not guarantees of a longer lifespan, age reversal, disease prevention, or any particular outcome. Results and experiences vary.
Alex is a board-certified family nurse practitioner and concierge provider. Care includes clinical evaluation, individualized recommendations, and follow-up within the agreed scope of services.
Location and access
Clinical services depend on the patient’s physical location at the time of care, provider authorization to practice there, the type of service, and clinical suitability. Hawaiʻi may support in-person and telehealth care; Illinois services are primarily telehealth, with local testing or outside referrals when appropriate. Some evaluations or procedures require an in-person visit. Availability must be confirmed before scheduling.
Concierge coordination
Our concierge team can help connect you with testing, imaging, and specialist services when appropriate. Coordination does not guarantee clinical suitability, availability, or membership inclusion. Personal preferences are discussed through secure intake, and any additional costs are confirmed before scheduling.
Inquiries and enrollment qualification
Inquiries are welcome regardless of coverage status. Sending a request does not establish eligibility for clinical services, book an appointment, or enroll a member.
A brief coverage question appears only in the DPC enrollment-review pathway. Every response, including yes, uncertain, or a preference to discuss with the team, can be submitted for individual review. Each prospective member’s clinical needs, patient location, coverage considerations, and applicable requirements must be evaluated before care or membership is confirmed.
For patients with Medicare, Medicare Advantage, or Medicaid/Med-QUEST, private payment alone does not establish eligibility or satisfy program requirements. The Longevity Concierge Med team must review the appropriate path before accepting clinical payment, confirming a clinical appointment, or enrolling a patient. No coverage response automatically approves or rejects a website inquiry.
Read Payment & Insurance for cash pay, DPC, outside costs, and qualification details.
Membership, fees, and outside services
Membership is not health insurance. Testing, medications, supplements, specialist care, imaging, and outside services may have separate fees unless expressly included in a signed agreement. Initial consultations are $149. The enrollment fee, included services, term, payment schedule, cancellation terms, and any discounts are confirmed before commitment.
A 15-minute discovery call is an introductory conversation with a concierge team member about services, membership, and logistics. It is not a clinical consultation and does not include diagnosis, prescribing, or medical advice. Discovery-call requests require separate confirmation of availability and a scheduled time.
Illustrative or proposed prices on this site are not a completed purchase. Submitting an inquiry does not reserve an appointment, verify a referral, enroll you, or create an automatic annual renewal. Signed clinical and membership documents govern the services to which they apply.
Website use, external links, and questions
Provide accurate information, use the website lawfully, and do not attempt to access another person’s information or disrupt the service. External resources and intake providers have their own terms and privacy practices. Information and availability can change; ask Longevity Concierge Med to confirm details relevant to your decision.
For questions, contact concierge@conciergemed.com. See the website privacy policy for information about website submissions. These website terms do not replace clinical privacy notices, treatment-specific informed consent, or a signed membership agreement.
