The website is operated by Regen NP LLC, doing business as Longevity Concierge Med, together with its affiliated brands and programs where expressly identified.

In these Terms:

  • “Longevity Concierge Med,” “we,” “our,” and “us” mean Regen NP LLC d/b/a Longevity Concierge Med. 
  • “Website” means ConciergeMed.com and the public-facing content and tools made available through it. 
  • “You” and “your” mean the person accessing or using the Website. 

By using the Website, you agree to these Terms and our Website Privacy Policy.

If you do not agree, do not use the Website.

It is not intended to replace individualized evaluation, diagnosis, treatment, or advice from a qualified healthcare professional who has evaluated you.

  • Obtains required consent and intake information 
  • Documents the relationship in the applicable medical record 

Submitting a form does not guarantee acceptance as a patient.


3. Not for Emergencies

The Website, email, social-media accounts, general text-message channels, and online forms are not emergency services and are not monitored continuously.

Do not use them to report:

  • Chest pain 
  • Difficulty breathing 
  • Severe allergic reactions 
  • Loss of consciousness 
  • Stroke symptoms 
  • Severe bleeding 
  • Suicidal thoughts 
  • Overdose 
  • Acute psychiatric crisis

4. Clinician Credentials and Practice Description

Longevity Concierge Med is founded and led by Alexander “Alex” Acevedo, FNP-C, a board-certified family nurse practitioner.


5. Scope of Practice and Geographic Limitations

Healthcare services are available only where the treating clinician:

  • Holds an active and appropriate professional license 
  • Is authorized to provide the particular service 
  • Can comply with applicable state law 
  • Has determined that telehealth or in-person treatment is clinically appropriate 

The location of the patient at the time of care may determine which state’s licensing and healthcare laws apply.

You agree to accurately disclose your physical location during telehealth encounters.

The Website’s availability in a state does not mean every service advertised is available in that state.

Certain medications, laboratory services, pharmacy services, regenerative procedures, telehealth services, or treatment options may be restricted by:

  • A clinician will agree with a treatment you requested 

All treatment decisions remain subject to independent clinical judgment.

A clinician may recommend:

  • A different treatment 
  • Additional testing 
  • Referral to another provider 
  • In-person evaluation 
  • Emergency evaluation 
  • No treatment 
  • Discontinuation of a therapy 


7. No Guarantees of Results

Healthcare outcomes vary.

Results may depend on factors including:

8. Emerging, Off-Label, and Non-FDA-Approved Uses

A compounded medication is not the same as an FDA-approved manufactured drug. The FDA generally does not verify a compounded medication’s safety, effectiveness, or manufacturing quality before it is dispensed in the same way it reviews an approved drug.

Discussion of a treatment on the Website does not constitute a recommendation or guarantee that it will be prescribed.

Do not submit detailed medical records, laboratory results, diagnoses, photographs of medical conditions, Social Security numbers, insurance identification numbers, full payment-card information, or other highly sensitive information through a public contact form unless the form is expressly designated as secure for that purpose.

We may redirect you to a secure intake platform or patient portal.

A Website form may be unavailable, delayed, misdirected, or affected by technical problems. Contact the clinic through another approved method when you do not receive a timely response.


11. Patient Portal and Secure Systems

Patient portals, intake platforms, telehealth systems, and electronic health records may be governed by additional:

Potential limitations include:

  • Inability to perform a complete physical examination

  • The need for in-person care 
  • The possibility that a service cannot legally be provided where you are located 

Telehealth care is governed by a separate Telehealth Informed Consent.


13. Text Messaging

Reply STOP to opt out of promotional text messages. Reply HELP for assistance.

Text messaging may not be fully secure. Avoid sending sensitive medical information through ordinary SMS unless specifically instructed.

Stopping promotional messages may not prevent legally permitted transactional or care-related communications sent through another authorized channel.


14. Email Communications

You may receive administrative, educational, service-related, or promotional email based on your relationship with us and the consent you provide.

You may unsubscribe from promotional email using the unsubscribe link.

Unsubscribing from marketing does not necessarily stop:

21. Memberships, Fees, Payments, and Refunds

Specific fees, membership benefits, renewal terms, cancellation requirements, and refund policies will be stated in the applicable:

22. Insurance and Reimbursement

Unless expressly stated otherwise, Longevity Concierge Med may operate as a direct-pay or cash-pay practice and may not participate with health insurance plans.

We do not guarantee that:

  • Your insurance will reimburse you 
  • A service is covered 
  • A laboratory or pharmacy is in network 
  • A submitted superbill will be accepted 
  • A payment will count toward a deductible 

You are responsible for confirming benefits and coverage directly with your health plan.


23. Privacy

Use of personal information collected through the public Website is governed by our Website Privacy Policy.

Use and disclosure of patient PHI are governed by our HIPAA Notice of Privacy Practices when HIPAA applies.

The two documents address different categories and contexts of information.

Information submitted to a general public website form may not automatically become part of a medical record or receive the same treatment as information submitted through an approved clinical system.



24. Children and Minors

The general Website is intended for adults.

We do not knowingly collect personal information directly from children under 13 through general marketing forms without appropriate parental authorization.

A parent, guardian, or legally authorized person must generally arrange care for a minor, except where law allows the minor to independently consent.

A parent or guardian should not submit information about a minor without authority to do so.

Any Website-related proceeding not involving a claim that must be brought elsewhere by law will be brought in a court of competent jurisdiction located in the City and County of Honolulu, Hawai‘i.

This provision does not prevent:

Termination of Website access does not automatically terminate an established provider-patient relationship. Any clinical termination must comply with applicable professional, ethical, and legal requirements.


33. Changes to These Terms

We may revise these Terms periodically.

Changes will be posted with a new “Last Updated” date.

Your continued use of the public Website after revised Terms are posted constitutes acceptance of the revised Terms, except where additional affirmative consent is legally required.

Changes to a signed membership, financial, or patient agreement will be handled under that agreement and will not occur merely through an update to these Website Terms.


34. Severability

If a provision of these Terms is found invalid or unenforceable, the remaining provisions will continue in effect to the fullest extent permitted by law.

The invalid provision will be interpreted or limited as closely as legally possible to its intended purpose.


35. No Waiver

Failure to enforce a provision does not waive the right to enforce it later.

A waiver must be in writing and signed by an authorized representative of Regen NP LLC.


36. Entire Agreement for Public Website Use

These Terms, together with the Website Privacy Policy and any notices expressly incorporated into them, constitute the agreement governing public Website use.

They do not replace:
